For any illness lasting seven days or less you do not require a doctor's sickness certificate. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC Website.

If you're off work sick for more than 7 days, your employer will usually ask for a fit note (or Statement of Fitness for Work) from a GP or hospital doctor. Fit notes are sometimes referred to as medical statements or a doctor's note.

To request a repeat sick note either

• complete an online request
• call the practice on 01642 477133.

Please state the date the request is due from, and length of time off required, for information we cannot post-date sick notes.

If you have already had a Sick Note (Fit Note) for this illness your Doctor may not need to see you to issue an additional Sick Note. Please complete this form. We will contact you to let you know when you can collect your Sick / Fit Note or we may contact you to arrange an appointment.
